Please fill out and submit the Student Enrollment Form.   You can bring the form into the office, or fax it to us.  In addition to the Student Enrollment Form, you will need to come into the office to fill out the Emergency Contact Card, and give us copies of your child's Birth Certificate and Immunization Records.

There are still some seats available in some grades, though space is limited.  To secure your child's spot, we need to receive the signed  Student Enrollment Form. We then need you to come into the office to finalize the additional paperwork.
